New Patient Information Packet
If this is your first time to see us in our new facility, please:
- Print the following forms on standard white 8.5" x 11" paper.
- Fill them out completely.
- Bring the completed forms with you to the clinic on the day of your appointment, or fax them to 713-366-7981, ATTN: Sharla Cooper or Jasmine Concepion, several days before your appointment.

- Notice of Privacy Practices – Please download and review.
- Authorization to Release Payment for Dr. Slawin’s patients (except patients with Medicare as their primary insurance) – Please download, print, fill out, and sign in ink.
- Authorization to Release Payment for Dr. Shu’s or Dr. Appell's patients and for all Medicare patients – Please download, print, fill out, and sign in ink.
- Medical History Questionnaire – Please download, print, and fill out completely.
- Release Form – Please download, print, fill out, and sign in ink if you are transferring your care to Dr. Slawin, Dr. Shu, or Dr. Appell from another practice.
- Specimen Bank Consent Form (Male patients only) – Please download, print, review, and sign if you agree to allow left-over serum and tissue or urine specimens obtained as part of your routine care to be retained in our specimen bank to support our research program. Please note that there are two places to answer questions and initial on page 2 and that your signature is required on page 4.
